With the exception of Population Health Risk Analysis, you may mix and match options to fit your individual needs or purchase the entire HealthReach program and receive the full program benefits.

Each Program Must Include

Population Health Risk
Analysis (HCS’ proprietary clinical predictive modeling technology) stratifies the member population to determine high risk individuals based on the following data elements:

Retrospectively identifies, reports and mitigates Rx risk due to duplicate therapies, dosage alerts, non-compliance with treatment plans and potentially life threatening drug-drug interactions through the generation of highly detailed communications to all providers.

My HealthReach Workbook
HCS’ 24/7 online resource engages patients with access to comprehensive health information, as well as, a portable Personal Health Record to track and meet care goals plus monitor new and existing prescription drugs

HealthReach Preventive Care
Reduces risk of acute illness and high cost claims through personalized communication to providers and patients. This program outreaches to individuals with two (2) or more evidence based gaps in care compared to HEDIS standards. Integrates with My HealthReach Workbook to help monitor compliance.

HealthReach Advocate
Direct member engagement for a unique HEDIS standard for each month of the year. Utilizes targeted mailings to contact individuals with gaps in those standards to help manage the high costs associated with chronic conditions.

HealthReach Coaching
HCS’ nurse staffed outbound coaching program designed to reach out to high risk individuals and educate those individuals on how to make better health decisions. In this program, a Registered Nurse works one on one with the member to identify and implement important lifestyle and treatment modifications.

Health Risk Assessment
Provides unlimited online access for all employees to Health Risk Assessment (HRA, Personal Wellness Report and Personalized Wellness Portal. Employers benefit from the online access to detailed aggregate reporting which includes the ability to compile data by a specific topic.
